Getting Ready for Your Visit

The better prepared you are, the better prepared we can be to take care of you! What to bring to your appointment: Please bring your insurance card and a photo ID. If you have any previous medical records please bring them with you. Dr. Schwartz will need a list of all your medications, the dose and the times you take them. You can print the form below. Registration: Please arrive at least 15 minutes before your appointment time to complete your registration, OR you may print the forms from this page (below) and bring them with you already completed. Payment: Be prepared to pay your deductibles, co-pays, co-insurance and any other amounts due at the time of your visit. Please remember that your appointment time is valuable, not only to you, but to Dr. Schwartz and the other patients being seen. Our front office staff will call you the day before your appointment to confirm. If you are unable to make your appointment please give us at least 24 hours notice so that someone else who needs care can be scheduled during that time. No-shows will be charged a fee. Dr. Schwartz will give you the time you need to discuss your concerns during your appointment. If you have to wait, we ask that you understand she is just giving that same compassion, consideration and care to another patient.
